Autocorrelation Maps for Optimal Cardiac Resynchronisation Therapy Setting

The study proposes parameters potentially suitable for optimal ventricular stimulation settings in cardiac resynchronization therapy. The parameters are determined from autocorrelation maps (ACM) calculated from surface potential maps of 17 patients, 14 of whom have left bundle branch block and three have right bundle branch block. Eleven of the patients with left bundle branch block responded positively to treatment (responders). In all patients, surface potential maps were measured during their spontaneous heart rhythm and with the device set to optimal settings for cardiac resynchronization therapy. The ACMs of the patients were compared with the ACMs of 33 healthy individuals using the Kolmogorov-Smirnov and Wilcoxon statistical tests. The ACM of the healthy group and the group of patients with left bundle branch block in spontaneous rhythm differ significantly (p < 0.00008) for both statistical tests using all 19 proposed parameters extracted from the ACM. Four of the parameters examined (Shannon entropy of the ACM value histogram, mean value, standard deviation, and geometric mean value of positive ACM values) showed a significant difference for responders between spontaneous heart activity and after optimal adjustment, while also showing similarity to the ACM of healthy individuals.
